By using the HopeMed’s online sites, products, and services, users (“You”, “Your”) agree that HopeMed may collect and process the following personal information that can be used to uniquely identify a user: name, age, civil status, gender, mailing and email address, contact numbers, and password. You may be asked to provide personal information through voluntary registration during the sign-in process, creation of web portal user access account, and submission of feedback and inquiry.

Risks Involved
Risk refers to the potential of an incident to result in harm or danger to a data subject or organization. Risks may lead to the unauthorized collection, use, disclosure, or access to personal data. It includes risks involving the confidentiality, integrity, and availability of personal data or the risk that processing will violate the general data privacy principles and the rights of data subjects. Physical records shall be disposed of through shredding, while digital files shall be erased from our computers and servers, or anonymized where appropriate. HopeMed has no obligation to keep personal information that a user makes available to other users through a platform other than the HopeMed software, such as in chat or other public functions. External websites and companies with links to and from HopeMed’s online sites and products may collect personal information about users when users visit their websites. HopeMed’s privacy policy does not extend to external websites and companies or third-party publishers with links to or from HopeMed’s products and online websites or who collect personal information through their on-line sites. Children Under the Age of 18
HopeMed will not knowingly collect personal information from any person under the age of 18. HopeMed encourages parents to instruct their children to never give out personal information when online. Parents who are concerned about the transfer of personal information from their children may contact us.
